   May 17th 10 AM -  3 PM
   May  24th 10 AM -  3 PM
   May 31st 10 AM – 3 PM

PLEASE BRING THE FOLLOWING INFORMATION  FOR YOUR CHILD’S REGISTRATION WITH YOU:

• Original Birth Certificate
• Social Security Card
• Immunization Records
• Picture I.D. of Custodial Parent or
Legal Guardian
• Proof of Residency (cable bill,
water bill, gas/electric bill)


YOUR CHILD MUST BE 5 YEARS OLD ON OR BEFORE AUGUST 1, 2013 TO BE ELIGIBLE FOR KINDERGARTEN FOR THE 2013/2014 SCHOOL YEAR
